Job Summary

Armada Family Practice is seeking a skilled and compassionate Social Prescribing Link Worker to join our team. As a key member of our primary care network, you will play a vital role in empowering individuals to take control of their health and wellbeing through referral to non-medical services.

Main Responsibilities
  • Provide personalized support to individuals, their families, and carers to improve their health and wellbeing.
  • Develop trusting relationships with people, creating a shared personalized care and support plan to address their needs.
  • Connect people to community groups, voluntary organizations, and statutory services to promote holistic care.
  • Work collaboratively with local agencies to raise awareness of social prescribing and reduce pressure on statutory services.
  • Provide regular updates and feedback to referral agencies, ensuring they are confident in the service and making appropriate referrals.

Requirements
  • NVQ Level 3 or equivalent qualifications.

  • Commitment to reducing health inequalities and proactively working to reach people from diverse communities.
