We are seeking a highly motivated and experienced Community Dietitian to join our team at St George's Healthcare NHS Foundation Trust. The successful candidate will be responsible for delivering a general service for adults within community dietetics clinics based in health centres across Wandsworth.
Main DutiesThe successful candidate will be responsible for:
- Delivering a general service for adults within community dietetics clinics based in health centres across Wandsworth.
- Managing a mixed caseload including nutrition support, IBS, diabetes, and weight management.
- Visiting patients in their own homes as part of a general domiciliary caseload.
- Working closely with the acute dietetics department at St George's Hospital in Tooting to ensure safe discharge and community follow-up.
This is an exciting opportunity to join a dynamic team of dietitians who are passionate about delivering high-quality patient care. As a Community Dietitian, you will have the opportunity to work with a range of patients and develop your skills in nutrition support, home enteral feeding, and diabetes management.
RequirementsTo be successful in this role, you will need:
- A BSc Postgraduate diploma in Nutrition and Dietetics.
- HCPC registration.
- Evidence of Continuing Professional Development (CPD).
- General postgraduate experience of working in NHS acute and/or community dietetic services.
