Job Title: Nurse Triage Coordinator

Description:

We are looking for a dedicated Nurse Triage Coordinator to join our team at University Hospitals Coventry and Warwickshire NHS Trust. As a key member of our healthcare team, you will be responsible for triaging patient referrals, applying clinical knowledge and risk assessment skills to ensure patients receive timely and effective care.

Key Responsibilities

  1. Undertake comprehensive holistic telephone or other virtual technology assessment of the patients presenting problem.
  2. Liaise with relevant healthcare professionals, relatives, and carers to acquire necessary clinical information to inform comprehensive triage of referral.
  3. Allocate referrals requiring an urgent response to the rapid response team to ensure timely patient care.
  4. Ensure practice is evidence-based through undertaking audit and critically appraising literature to achieve a high standard of care.
