Job Summary

We are seeking a highly motivated and innovative Registered General Nurse to lead an integrated community nursing team under the guidance of the Modern Matron and Senior Community Lead Nurse in Leigh on sea.

About the Role

As a Community Nursing Team Lead, you will be responsible for the ongoing assessment, development, implementation, and monitoring of healthcare services to ensure the highest standard of service and care is delivered in a consistent and safe manner, within the home and clinical setting.

You will undertake a range of clinical duties/nursing procedures and participate in the development of guidelines and protocols, ensuring all policies are implemented and adhered to.

Key Responsibilities
  • Lead a dedicated nursing service to include assessment, planning, treatment, and delivery of nursing care in the community.
  • Assess health needs, lead a team of staff, and meet the ever-developing role of the community nurse.
  • Provide patient support to prevent unnecessary hospital admission, enabling them to stay at home where possible.
  • Supervise, support, and teach junior staff.
  • Liaise closely with multi-disciplinary teams as part of collaborative working to support the case management of patients.
Person Specification

Education/Qualifications

  • RGN first-level nurse.
  • Willingness to undertake Extended & Supplementary Nurse Prescribing Course.

Desirable

  • District Nursing Diploma/Degree.

Knowledge

  • Community experience.

Skills

  • Good oral communication skills based on fluency in the English language.
  • Literate in IT/Computer Skills.
