Discharge Coordinator

We are seeking a highly organized and detail-oriented individual to join our team as a Discharge Coordinator. As a key member of our multidisciplinary team, you will play a vital role in ensuring the smooth transition of patients from our hospital to their next level of care.
Main Responsibilities- Act as the primary point of contact for discharge information, providing timely and accurate updates to patients, families, and healthcare professionals.
- Collaborate with the multidisciplinary team to develop and implement effective discharge plans, ensuring seamless transitions and minimizing delays.
- Communicate regularly with ward staff, bed coordinators, and other stakeholders to ensure timely bed availability and efficient patient flow.
- Facilitate the transfer of patients to other wards or facilities, ensuring all necessary documentation and information are completed and up-to-date.
- Participate in ward rounds, nurse handovers, and multidisciplinary team meetings to stay informed and contribute to patient care.
- Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare professionals.
- Strong organizational and time management skills, with the ability to prioritize tasks and manage multiple projects simultaneously.
- Ability to maintain accurate and up-to-date records, with a high level of attention to detail.
- Knowledge of patient confidentiality and data protection principles.
- Computer literacy and proficiency in using IT systems, with the ability to learn new software and technology.
