Job Overview:

Unity Healthcare Recruitment Ltd is seeking a skilled and compassionate community nurse to join their team. This role involves delivering high-quality nursing care to patients in their own homes or community settings, promoting independence and achieving healthcare goals.

Key Responsibilities:
  • Clinical Care:
    • Deliver a range of nursing interventions, including wound care, medication administration, catheter care, and end-of-life care.
    • Conduct holistic assessments of patients' physical, emotional, and social needs to develop and implement individualised care plans.
    • Monitor and evaluate patients' progress, adapting care plans as necessary to achieve the best outcomes.
    • Provide health education and advice to patients and carers to support self-management and promote independence.
  • Community-Based Care:
    • Work autonomously in patients' homes and community settings, ensuring safe and effective care delivery.
    • Manage a daily caseload, prioritising tasks and responding to changes in patients' conditions.
    • Use digital tools and systems to plan, record, and report on care delivery in line with organisational policies.
  • Team Collaboration:
    • Work closely with other healthcare professionals, such as GPs, therapists, social workers, and voluntary sector partners, to ensure coordinated care.
    • Participate in multidisciplinary team meetings and case conferences to support care planning and review.
    • Act as a patient advocate, ensuring their preferences and needs are central to care planning and decision-making.
  • Professional Development:
    • Commit to continuing professional development (CPD) to maintain and enhance clinical skills and knowledge.
    • Participate in clinical supervision, reflective practice, and training sessions provided by the organisation.
    • Support the development of students, healthcare assistants, and less experienced colleagues through mentoring and guidance.

Salary: £30,000 - £42,000 per annum
