Job Title: Community Diabetes Healthcare Assistant - Surrey Downs H&C

We are seeking a highly motivated and compassionate Community Diabetes Healthcare Assistant to join our team at Surrey Downs Healthcare. As a key member of our Diabetes Specialist Team, you will play a vital role in providing patient education and support to individuals with diabetes.

Key Responsibilities:
  • Communication and Engagement: Provide support to patients to help them make informed decisions about their care, including face-to-face, telephone discussions, and responding to email queries.
  • Diabetes Education: Provide individualized diabetes care tailored to patient needs and circumstances, including education on hypoglycemia management, insulin initiation, and completion of the Gold score questionnaire.
  • Clinic Support: Ensure smooth running of clinics by supporting clinicians with duties such as blood pressure monitoring, weight measurement, and blood glucose or ketone monitoring.
  • Patient Support: Support patients to download their glucometer data onto Diasend before their appointment time and liaise with patients regarding annual health checks to improve patient safety.
  • Health Promotion: Support the diabetes team with health promotion and awareness events, including sourcing promotional materials, organizing venues and stands, advertising the events, and engaging with the public.
Person Specification:
  • Qualifications: Good general education and ability to demonstrate literacy and numeracy skills.
  • Desirable Criteria: NVQ Health and Social Care level 3 or equivalent, EMIS IT systems knowledge, experience in working as part of a diabetes team, diabetes technologies experience, and quality improvement.
