Community Diabetes Healthcare Assistant

Job Overview:
We are seeking a highly motivated and dedicated Community Diabetes Healthcare Assistant to join our team at Surrey Downs Health and Care. As a key member of our Diabetes Specialist Team, you will play a vital role in providing patient education and support to individuals with diabetes.
Main Responsibilities:
- Provide individualized diabetes care tailored to patient needs and circumstances.
- Support patients to make informed decisions about their care through face-to-face, telephone discussions, or email queries.
- Deliver diabetes education to patients, covering topics such as hypoglycaemia management, insulin initiation teaching, and Gold score questionnaire completion.
- Ensure smooth running of clinics by supporting clinicians in various duties, including blood pressure monitoring, weight measurement, and blood glucose or ketone monitoring.
- Liaise with patients regarding annual health checks to improve patient safety.
- Support diabetes team with health promotion and awareness events, such as Insulin Safety Week, Hypo Awareness Week, and World Diabetes Day.
- Provide education to patients on how to use blood glucose and ketone meters.
- Ensure education materials are accessible to patients, requiring liaison with company representatives for leaflets, study materials, and demo kits.
- Support the diabetes team in delivering Freestyle Libre and Bertie educational sessions.

- Good general education
- Able to demonstrate literacy and numeracy skills
Desirable Criteria:
- NVQ Health and Social Care level 3 or equivalent
Knowledge:

- EMIS IT systems Knowledge
- Experience in working as part of a diabetes team
- Diabetes Technologies experience
- Quality Improvement

- Patient-facing roles - able to understand an individual's communication, physical, and emotional needs; recognise their needs and preferences
