About the Role
This role offers an exciting opportunity to join our team as a Community Care Worker in CC / South / Sussex. As a valued member of our team, you will be responsible for delivering high-quality care and support to our clients, ensuring their physical, emotional, and social needs are met.
Key Responsibilities
- Deliver person-centred care, including personal care, companionship, and medication management
- Develop and maintain strong relationships with clients, families, and healthcare professionals
- Work collaboratively with colleagues to ensure seamless delivery of care services
- Maintain accurate records and reports, adhering to regulatory requirements
Requirements
- A minimum of 6 months of professional care experience or equivalent in caring for a loved one in the UK
- Ability to work independently and as part of a team
- Right to work in the UK
- Experience with complex care (desired but not essential)
