As a Complex Care Coordinator, you will play a crucial role in ensuring the seamless delivery of care services to children with complex needs in their own homes. You will work collaboratively with Complex Care Managers (CCMs), other clinical professionals, and support workers to implement timely remediations, monitor and audit clinical paperwork, and complete quarterly joint clinical and operational visits.

Your key responsibilities will include:* Coordinating new client package set-ups, including clinical assessments and training of support workers* Liaising regularly with clients and support workers to review care packages and make necessary alterations for continuation of care* Monitoring and auditing the completion of clinical paperwork, ensuring errors are followed up and resolved appropriately* Completing quarterly joint clinical and operational visits, documenting visit reports and following up on concerns identified* Maintaining monthly reviews with ICBs and developing and maintaining cross-work with other areas of the wider business* Participating in professional development discussions with your line manager to align personal growth goals with organisational needs and career advancement* Providing specialist care and managing complex cases, including advanced procedures and multi-disciplinary interventions* Managing own diary effectively, scheduling appointments, patient visits, and other professional commitments independently* Possessing a valid UK driving license and access to a reliable vehicle for travel between locations* Travelling up to 2 hours within specified regions to provide care, attend meetings, and engage in other professional activities* Working outside standard hours or taking on additional responsibilities as needed to meet patient and organisational needs

The successful candidate will have extensive experience working in Intensive Care Units (ICU), spinal care units, or with complex care patients, demonstrating a high level of expertise in handling severe and multifaceted medical conditions. They will also possess strong organisational skills, the ability to effectively schedule and manage appointments, patient visits, and other professional commitments independently, and comprehensive knowledge of age-specific care practices, treatment plans, and communication strategies for effective patient management.
