**Job Description**
">We are looking for a highly skilled NHS Advanced Practitioner to join our team at Primary Care Physio. As a key member of our team, you will play a vital role in supporting NHS Primary Care Networks and their GP practices in delivering high-quality and tech-enabled NHS primary care services.
Your primary responsibilities will include assessing, diagnosing, triaging, and managing patients, developing and utilizing your full scope of clinical practice, and managing complex interactions. You will also deliver programmes of supported patient self-management, communicate effectively with patients and carers, and implement effective clinical governance.
To succeed in this role, you will need to have a minimum of 5 years post-graduate physiotherapy experience, including 3 years of relevant MSK clinical experience. You should be flexible to the changing fast-paced nature of working in primary care and able to rise to the challenges it presents. A fully HCPC and CSP registered practicing Physiotherapist with non-medical prescribing qualifications and a Disclosure and Barring Service Check are essential requirements.
