Job DescriptionThis challenging yet rewarding role involves working closely with a multi-disciplinary team to provide specialist support and comprehensive geriatric assessments to patients in their own homes and across the acute site. As an ANP, you will use your advanced clinical reasoning skills to identify and manage frailty patients, promoting admission avoidance and supporting the Hospital@Home model.
The successful candidate will have substantial experience as a Band 6 in an acute medical setting and possess independent prescribing qualifications or be willing to undergo training. You will also have experience of working in an ambulatory emergency care setting and possess substantial specialist experience of frail patients.
We offer a highly competitive salary of £46,148-£52,809 per annum, pro rata, and a comprehensive benefits package, including access to ongoing professional development opportunities.
