Community Health Practitioner

We are seeking an experienced and skilled Community Health Practitioner - District Nursing to join our team at Cwm Taf Morgannwg University Health Board.
You will have a strong background in nursing and be able to demonstrate confidence and competence in a wide range of clinical skills, including palliative care, chronic disease management, and wound management.
In this role, you will work as part of a multi-disciplinary team to provide holistic care to patients in their own homes, prioritizing patient-centered care and promoting independence.
