Job DescriptionThe successful candidate will be responsible for providing holistic assessments of patient needs and developing evidence-based programmes of care. You will work autonomously in relation to patient nursing needs, exercising professional judgment and decision-making skills. This role requires strong leadership and management abilities, as well as experience in managing teams and service development.
You will assist the Lead Advanced Nurse Practitioner in clinical assessments and work alongside medical and GP colleagues to maintain patients in a community setting, avoid hospital admissions, and facilitate discharges. A key aspect of this role is collaboration with Health and Social Care staff on reablement, avoidance of hospital admission, and early discharge facilitation.
This position demands a full understanding of relevant care pathways and the ability to work effectively with Health and Social Care staff on evolving service transitions. The successful candidate will be expected to exercise initiative in differentiating between issues requiring referral and those that can be addressed through appropriate decision-making.
