Job Summary

United Lincolnshire Hospitals NHS Trust is seeking an experienced Children's Community Nurse to join our Lincolnshire Childrens Nursing (CCN) Service. The successful candidate will have a passion for community childrens nursing and a commitment to providing high-quality care to children and their families.

About the Role

The Children's Community Nurse will work as part of a multidisciplinary team to provide nursing care to children with acute and chronic health conditions in their own homes. This will involve working closely with children, their families, and other healthcare professionals to develop and implement individual care plans that meet the needs of each child.

Key Responsibilities

  • Provide nursing care to children with acute and chronic health conditions in their own homes
  • Develop and implement individual care plans that meet the needs of each child
  • Work closely with children, their families, and other healthcare professionals to provide comprehensive care
  • Participate in the out-of-hours telephone on-call system to support all children and families on the CCN countywide caseload
  • Contribute to the development of ambulatory care and promote the ethos of empowering children and their families to become self-caring, independent, and manage their own condition at home wherever possible

Requirements

  • NMC Registered Nurse (Child)
  • Recent experience of working as a children's nurse
  • Experience of safeguarding children / child protection
  • Ability to travel independently throughout the County
  • Flexible working
  • Commitment to developing children's community services
  • Commitment to participate in a county-wide CCN caseload on-call rota 5pm to 10pm weekdays & 9am to 10pm at weekends and Bank Holidays, including participation in a 24/7 on-call rota for children & young people requiring end of life care
