About the Role
We are seeking an experienced Childrens Community Nurse to join our team in Lincolnshire. As a key member of our Community Childrens Nursing Team, you will be responsible for providing high-quality nursing care to children and young people in their own homes.
Key Responsibilities
- Formulate individual care packages to facilitate the smooth transition from hospital to home.
- Manage your own caseload and coordinate services to reduce hospital re-admissions and provide support and nursing care to children and young people with acute healthcare needs.
- Participate in the out-of-hours telephone on-call system to support all children and families on the CCN countywide caseload.
Requirements
- NMC Registered Nurse (Child)
- Recent experience of working as a children's nurse
- Ability to travel independently throughout the County
