Clinical Lead for Virtual Frailty Ward and REACT Service

About the Role
The East Suffolk and North Essex NHS Foundation Trust is seeking a skilled and dedicated Clinical Lead to join our team in the Virtual Frailty Ward and REACT Service. As a key member of our multidisciplinary team, you will be responsible for providing medical reviews of patients in their own home environment, planning and managing episodes of care, and working in partnership with healthcare professionals to optimize health outcomes and resource use.
Key Responsibilities
- Provide medical reviews of patients in their own home environment
- Plan and manage episodes of care
- Work in partnership with healthcare professionals to optimize health outcomes and resource use
- Participate in daily morning handovers and twice-weekly multidisciplinary team meetings
- Collaborate with senior medical cover and consultant colleagues
Requirements
- Full registration with the GMC
- MBBS or equivalent
- MRCP part 1
- Experience at CMT/VTS level of acute geriatric and general medicine
- NHS experience
- Ability to organize and prioritize competing demands
- Flexibility to respond to changing service needs
- Valid driving license
