**Key Responsibilities**

  • Develop and implement personalized care plans in collaboration with residents, families, and healthcare professionals.
  • Lead and manage a team of care staff, providing guidance, support, and coaching to ensure high-quality care delivery.
  • Conduct regular assessments and reviews to ensure residents' needs are met and care plans are updated accordingly.
  • Collaborate with other departments, such as nursing, therapy, and activities, to ensure a holistic approach to care.
  • Ensure compliance with regulatory requirements, company policies, and procedures.

**Requirements**

  • Minimum NVQ Level 3 in Care or equivalent qualification.
  • Proven experience in a care leadership role, preferably in a residential care setting.
  • Excellent communication, interpersonal, and leadership skills.
  • Ability to work autonomously and as part of a team.
  • Knowledge of current Person Centred Care planning practice and delivery.
