Job Title: Adult Mental Health Practitioner


Job Summary: We are seeking a qualified and experienced Adult Mental Health Practitioner to join our Integrated Community Mental Health Team.


Main Duties:



  • Assess care needs and develop, implement, and evaluate programmes of care for a defined clinical caseload.
  • Work as a Care Co-ordinator within Care Treatment Planning under the Mental Health (Wales) Measure 2010.
  • Communicate effectively with healthcare professionals, statutory and voluntary agencies, and maintain good communication across all boundaries.

Person Specification:



  • Professional qualification/registration from a recognised body (e.g., RMN, Occupational Therapist, or Social Worker).
  • Post-basic education in the area of practice to postgraduate diploma level.
  • Wide range of experience within a service-related setting.
  • Demonstrates achievements in developments of care standards/procedures guidelines.
  • Demonstrates effective team working.
  • Use of I.T. skills.
  • Demonstrates research and audit experience.

Language Skills:



  • Welsh Speaker (Level 1).

This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service (formerly known as CRB) to check for any previous criminal convictions.


About the Role: The successful applicant will join the Integrated Community Mental Health Team, based at Wellfield Resource Centre, Carmarthen. The post is permanent and will be full-time - 37.5 hours per week. The hours will be worked in 7.5hr shifts over a 9am to 5pm shift pattern rota, Monday to Friday.
