Job Summary

As a Senior Social Care Specialist at Invest Solutions Limited, you will play a vital role in delivering high-quality care services to our clients. Your primary responsibility will be to complete social care assessments and undertake casework of residents in the community and other care settings. You will commission care services according to assessment outcomes and eligibility in relation to the Care Act. Additionally, you will participate in the duty rota, serving as the single point of contact for referrals for Adult Social Care. In situations where immediate risk management is necessary, you will undertake crisis intervention.

Key Responsibilities

• Complete social care assessments and undertake casework of residents in the community and other care settings.
• Commission care services according to assessment outcomes and eligibility in relation to the Care Act.
• Participate in the duty rota, serving as the single point of contact for referrals for Adult Social Care.
• Undertake crisis intervention where necessary.
• Carry out home visits and consult with service users, carers, and relevant agencies and professionals.
• Alert the Registered Care Manager to any safeguarding concerns.
• Ensure assessments and support plans are carried out in a person-centered and outcome-focused way.
• Advise on the use of personal budgets and direct payments.
• Assist customers to maximise their independence, where appropriate.
• Undertake any necessary adjustments or cease care packages in accordance with both the organization's Eligibility Criteria and Operational Guidance and the needs of the service user.
