About the Role

We are seeking a highly skilled and compassionate Care Home Matron to join our team at Blackpool Teaching Hospitals NHS Foundation Trust. As a key member of our integrated Neighbourhood, you will work closely with care homes to support residents with complex needs and long-term conditions.

Key Responsibilities

  • Plan, manage, and coordinate care for residents with highly complex needs and long-term conditions.
  • Work collaboratively with care homes, acute hospitals, and community teams to ensure residents receive the right care, at the right time, in the right place.
  • Support the development of enhanced care in care homes, including proactive work around anticipatory care plans.
  • Provide training and support to care homes as needed.

Requirements

  • First-level registered nurse or healthcare professional with current registration with the NMC or HCPC.
  • Ability to work within PGDs.
  • Completion of CPD module at level 6 in clinical investigation skills.
  • Leadership training (LEO/LPC) or equivalent experience.
  • Non-medical prescribing qualification V300.
