About the Role

We are seeking a highly skilled and compassionate Care Home Matron to join our team at Blackpool Teaching Hospitals NHS Foundation Trust. As a key member of our integrated Neighbourhood, you will work closely with care homes and other healthcare professionals to provide high-quality care to residents with complex needs.

Key Responsibilities
  • Plan, manage, and coordinate the care of residents with highly complex needs and long-term conditions.
  • Work collaboratively with care homes, acute hospitals, and community teams to ensure residents receive the right care, at the right time, in the right place.
  • Support the development of Enhanced Health in Care Homes strategy and work with care homes to deliver enhanced care.
  • Provide training and support to care homes as needed.
  • Work with multidisciplinary teams to identify trends and analyze data to improve care outcomes.
Requirements
  • First-level Registered Nurse or Healthcare Professional with current registration with the NMC or HCPC.
  • Ability to work within PGDs.
  • Completion of CPD module at level 6 in clinical investigation skills.
  • Leadership training (LEO/LPC) or equivalent experience.
  • Non-medical prescribing qualification V300.
Desirable Qualifications and Experience
  • Degree or module in a health-related topic.
  • Teaching qualification.
  • Advanced communication module.
  • Ability to complete DNARCPR when required.
