We are seeking a highly skilled and compassionate Care Home Matron to join our team at Blackpool Teaching Hospitals NHS Foundation Trust. As a key member of our integrated Neighbourhood, you will work closely with care homes and other healthcare professionals to provide high-quality care to residents with complex needs.
Key Responsibilities- Plan, manage, and coordinate the care of residents with highly complex needs and long-term conditions.
- Work collaboratively with care homes, acute hospitals, and community teams to ensure residents receive the right care, at the right time, in the right place.
- Support the development of Enhanced Health in Care Homes strategy and work with care homes to deliver enhanced care.
- Provide training and support to care homes as needed.
- Work with multidisciplinary teams to identify trends and analyze data to improve care outcomes.
- First-level Registered Nurse or Healthcare Professional with current registration with the NMC or HCPC.
- Ability to work within PGDs.
- Completion of CPD module at level 6 in clinical investigation skills.
- Leadership training (LEO/LPC) or equivalent experience.
- Non-medical prescribing qualification V300.
- Degree or module in a health-related topic.
- Teaching qualification.
- Advanced communication module.
- Ability to complete DNARCPR when required.
