About the Role

We are seeking a highly skilled and compassionate Care Home Matron to join our team at Blackpool Teaching Hospitals NHS Foundation Trust. The successful candidate will work within the Enhanced Health in Care Homes strategy, supporting care home residents with complex needs and long-term conditions.

Main Responsibilities

  • Plan, manage, and coordinate the care of individuals with highly complex needs and long-term conditions in care homes.
  • Work collaboratively with care homes, acute hospitals, and community teams to ensure residents receive the right care at the right time in the right place.
  • Support the development of anticipatory care plans and provide training and support to care homes as needed.

Requirements

  • First-level registered nurse or healthcare professional with current registration with the NMC or HCPC.
  • Ability to work within PGDs and completion of CPD module at level 6 in clinical investigation skills.
  • Leadership training (LEO/LPC) or equivalent experience.
  • Non-medical prescribing qualification V300.
