We are seeking a skilled and motivated Mental Health Nurse to join our established integrated, multi-disciplinary team in North Wales. The successful candidate will be responsible for the care co-ordination and case management of a defined caseload of clients with complex and challenging needs.
Key Responsibilities- Undertake responsibility for the care co-ordination of a designated caseload of clients with severe and enduring mental health problems.
- Ensure that Care and Treatment Plans are adhered to and that standards are met in line with the Mental Health Measure (Wales).
- Act as a point of contact for GP's and other interface services to offer advice, support, and signposting to appropriate agencies.
- Formulate and develop standard and complex outcome-focused care and treatment plans.
- Ensure that a comprehensive risk assessment is completed and documented for actual and potential risk.
- Support clinical duties such as assisting with the running of the depot/clozapine clinic and the administration and supervision of long-acting injections.
- Complete physical health assessments and side-effect monitoring for patients prescribed Clozapine.
- Assess individuals in Crisis, complete comprehensive assessments of an individual's health and social care needs, and agree the most appropriate course of action.
- Assess, evaluate, and manage clinical risk, formulate a plan of care and interventions based on identified need, and deliver evidence-based crisis interventions as appropriate.
