Job Title: Senior Palliative Care Social Worker

About the Role:

Finegreen is currently supporting a local hospice that provides free, compassionate palliative care and support to patients and their families. The hospice has expanded its service provision in recent years, working more in the community as well as providing hospice-based care. They are now looking to complement their existing team with a senior-level qualified social worker who has experience working in similar settings within the community.

Key Responsibilities:

* Triage referrals into the service and undertake assessments of needs
* Allocate to appropriate support, including referral to specialist services where additional mental health or other needs are identified
* Attend in-house weekly IPU and FSS MDTs, and other in-house and community-based specialist palliative care multi-disciplinary team meetings when required
* Support the Family Support Service Manager in the update and debrief of FSS Spiritual Care Volunteers visiting weekly on IPU
* Assist in planning and implementing systems for evaluation, monitoring, and development of the role of the social worker in supporting patients with life-limiting illnesses and their families/carers

Requirements:

* Professional experience of external Social Care systems and pathways
* Knowledge of legislation and national guidance in relation to social work and how this is applied to best support issues faced by patients and families in end-of-life care and bereavement care
* Knowledge of relevant legislation and best practice relating to safeguarding patient/client safety, safeguarding adults at risk, mental capacity, and deprivation of liberty safeguards, human rights, and the Children's Act
