About the Role

We are seeking a highly skilled and compassionate Palliative Care Specialist to join our team at Garden House Hospice Care. As a key member of our nursing team, you will play a vital role in delivering high-quality care to patients with life-limiting illnesses and their families.

Key Responsibilities
  • Provide specialist palliative care services to patients with life-limiting illnesses, including symptom management and end-of-life care.
  • Support and lead the ward team in delivering high-quality care, ensuring adherence to the NMC Code of Conduct and Hospice Values.
  • Collaborate with multidisciplinary teams to develop and implement personalized care plans, including advance care planning and treatment escalation planning.
  • Supervise and mentor staff nurses, APs, NAs, and HCAs to ensure the delivery of direct and indirect care.
Requirements
  • Minimum of 2 years' experience in life-limiting disease management or palliative and end-of-life care.
  • Experience in team and ward management, including leadership and mentoring.
  • Ability to assess complex patient needs and develop and implement appropriate nursing interventions.
  • Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare professionals.
  • Computer skills, including the use of electronic patient record systems.
