About the Role

We are seeking a highly skilled and compassionate Social Prescribing Link Worker to join our team at Provide CIC. As a key member of our community outreach team, you will play a vital role in supporting patients and their families to access community resources and services that promote health and wellbeing.

Key Responsibilities
  • Community Outreach and Engagement
    • Build strong relationships with local community groups, organizations, and stakeholders to identify and develop opportunities for social prescribing.
    • Work collaboratively with healthcare professionals to promote social prescribing and its benefits to patients and their families.
  • Patient Support and Navigation
    • Provide personalized support to patients and their families to help them navigate the complexities of the healthcare system and access community resources and services.
    • Work with patients to identify their strengths, goals, and priorities, and develop personalized support plans to promote health and wellbeing.
  • Data Capture and Evaluation
    • Collect and analyze data to track the impact of social prescribing on patient outcomes and healthcare utilization.
    • Contribute to the development and implementation of quality improvement initiatives to enhance the effectiveness of social prescribing services.
Requirements
  • Education and Experience
    • Degree in a relevant field (e.g., social work, nursing, public health) or equivalent experience.
    • Proven experience in community outreach, patient support, and social prescribing or a related field.
  • Skills and Competencies
    • Excellent communication and interpersonal skills.
    • Ability to work collaboratively with diverse stakeholders, including patients, families, healthcare professionals, and community organizations.
