Job Summary

An exciting opportunity has arisen to join our innovative home nursing team, providing high-quality care and support to patients in their own homes.

We are seeking a highly skilled and experienced Community Care Leader to lead our team of nurses in delivering excellent patient care, promoting independence, and enhancing the quality of life for our patients.

About the Role

The successful candidate will have a proven track record in community nursing, with excellent clinical skills, leadership abilities, and a passion for delivering person-centred care.

You will be responsible for leading a team of nurses in providing comprehensive care and support to patients in their own homes, including assessment, planning, implementation, and evaluation of care.

In addition, you will work closely with other healthcare professionals to ensure seamless transitions between hospital and community settings, promoting continuity of care and reducing readmissions.

Key Responsibilities
  • To provide high-quality, patient-centred care and support to patients in their own homes
  • To lead a team of nurses in delivering excellent patient care, promoting independence, and enhancing the quality of life for patients
  • To assess, plan, implement, and evaluate care for patients in accordance with their individual needs and preferences
  • To work closely with other healthcare professionals to ensure seamless transitions between hospital and community settings
Requirements

The ideal candidate will possess:

  • RGN qualification and current NMC registration
  • At least 2 years' experience in community nursing
  • Excellent clinical skills, leadership abilities, and a passion for delivering person-centred care
  • Ability to demonstrate good leadership skills, advanced nursing skills, and effective people management skills

A competitive salary of £45,000 - £60,000 per annum, depending on experience, plus benefits package.
