SEN Case Officer Role

Connect2Socialwork is seeking a dedicated SEN Case Officer to join Surrey on a locum basis. As an SEN Case Officer, you will work with families of children with Special Needs and Disability in a highly person-centered relational approach, coordinating service provision across education, health, and care to ensure holistic delivery plans for individual children and families.

Key Responsibilities:

Work collaboratively with families, schools, and other key partners to ensure children and young people are educated and receive appropriate provision that meets their holistic needs.

Facilitate engagement with educational settings and relevant practitioners to support, advise, and monitor the local offer and work with schools on individual cases as needed.

Plan production and the experience of families, carers, children, and young people should be of high quality, with the use of data and feedback driving quality improvements.

Requirements:

Drive and have access to a car.
