Job Description:

We are seeking a skilled Substance Misuse Support Worker to join our Forward Leeds Focussed Intervention Team. The ideal candidate will have experience working with individuals facing issues related to alcohol and/or drugs, providing structured support through needs assessment and care planning.

Responsibilities:

  1. Assess client needs and develop care plans to achieve treatment goals.

  2. Deliver one-to-one interventions and provide ongoing support.

  3. Collaborate with the team to ensure comprehensive support for clients.

Requirements:

  • Experience working with individuals with substance misuse issues.

  • Ability to assess client needs and develop effective care plans.

  • Excellent communication and interpersonal skills.
