About the Role

We are seeking a skilled and compassionate Palliative Care Nurse to join our team at St Davids Hospice. As a Night Shift Nurse, you will provide high-quality care to patients with palliative care needs, working in collaboration with our multidisciplinary team.

Main Responsibilities
  • Provide palliative care to patients in their own homes, focusing on symptom management and quality of life.
  • Work closely with patients, families, and carers to develop personalized care plans.
  • Monitor patients' conditions and take appropriate action to ensure their comfort and well-being.
  • Collaborate with other healthcare professionals to ensure seamless care coordination.
  • Participate in clinical supervision and hospice at home meetings to stay up-to-date with best practices.
Requirements
  • RGN 1 qualification and NMC registration.
  • Experience in caring for patients with palliative care needs.
  • Community experience and knowledge of the Care Decisions Tool for the last days of life.
