Care Team Coordinator Job Description

As a Care Team Coordinator at Home Support Matters, you will play a vital role in the day-to-day planning of our care team's workload.

Key Responsibilities:

  • Efficiently allocate care workers to customer visits, considering effective rostering practices.
  • Support the care team in their daily tasks, ensuring they adhere to best practices.
  • Assist Field Care Supervisors through effective communication, enabling timely risk assessments.
  • Act as the primary contact for new referrals from private customers and social services teams.
  • Record, investigate, and respond to complaints within specified timelines.
  • Monitor and respond to Live Alerts promptly during the working day.
  • Oversight of care workers to ensure they match customer requirements.
