Nursing Support Professional in Community Setting

East Lothian Hospital to Home Team requires a skilled and caring Nursing Support Professional to join their team. As part of a new initiative, we are creating a dedicated team of Health Care Support Workers to deliver high-quality care to patients in their homes.
The successful candidate will assess, plan, provide, manage, monitor, and evaluate nursing care, following best practice to meet patient needs in a community setting. Responsibilities include assistance with daily living activities, medication management, meal preparation, end-of-life care, and palliative care.
As a member of the team, you will also support patients with qualifications such as Basic Literacy Skills, Good Basic Education, English Language Competency, and Communication Skills. A valid driving license is required for this role.
We anticipate the salary for this position to be approximately £20,000-£25,000 per year, considering factors like location, experience, and industry standards. Join us in making a difference in the lives of patients and their families.
