Community Care Professional

Do you have a passion for delivering high-quality patient care services in the community? We are seeking a dedicated and experienced nurse to join our team at Leaders In Care Recruitment Ltd as a Community Care Professional. As a key member of our community care team, you will be responsible for providing face-to-face support to adult clients and their families, while creating personalized care plans that cater to their unique needs.

This is an excellent opportunity for a registered general nurse (RGN) with experience in tracheostomy care and ventilators to showcase their clinical skills and make a real difference in people's lives. You will work closely with a core team of carers, training and supporting them with their clinical skills and activities. If you have up-to-date experience and a strong commitment to delivering high-quality patient care, we encourage you to apply.

Key Responsibilities:
  • Provide face-to-face support to adult clients and their families
  • Develop and implement care plans that meet the unique needs of each client
  • Collaborate with community teams, CHC nurses, and CCG commissioners to establish and maintain bespoke packages of care

• Competitive salary range of £45,000 - £55,000 per annum, depending on experience
