Job Overview

We are seeking a highly skilled and compassionate Community Mental Health Nurse to join our Assessment Team at Bradford District Care NHS Foundation Trust.

Main Responsibilities
  • Manage a caseload of clients in the community, providing high-quality care and support to individuals with mental health needs.
  • Triage and assess routine referrals into the Community Mental Health Assessment Team, ensuring timely and effective interventions.
  • Input data into the clinical information system, including risk assessments, care plans, and crisis plans.
  • Work as part of a duty/day coordinator service within the Community Mental Health Assessment Team, providing seamless care coordination.
  • Supervise and mentor student nurses, promoting their professional development and growth.
  • Demonstrate effective networking skills, ensuring a holistic approach to meet service user needs and promoting collaborative working with other agencies.
  • Ensure the safety and well-being of service users and their families, providing a safe and supportive environment.
  • Work in partnership with service users and carers to achieve goals and support recovery and reintegration within communities.
  • Provide appropriate feedback and liaison with the multidisciplinary team on all aspects of service user care.
  • Ensure that agreed standards of nursing care within the team are achieved, promoting excellence in care delivery.
  • Participate in flexible formats of team supervision and support, ensuring effective communication and collaboration.
  • Initiate, develop, and assist in the trial of new ideas for the improvement of services to service users in conjunction with the multidisciplinary team.
  • Ensure that nursing policies are implemented and maintained, promoting a culture of quality and safety.
  • Comply with legal and professional requirements, ensuring adherence to regulatory standards.
  • Identify, facilitate, and support developments and initiatives within the service delivery of nursing and multidisciplinary teams.
  • Work within a fully integrated and jointly managed Community Mental Health Team, alongside Local Authority social care staff and managers.

Person Specification

We are seeking a candidate with the following qualifications and experience:

  • Current full UK, NMC Registration
  • At least two years post-registration nursing experience
  • Experience of working in a community mental health setting (desirable)
  • Excellent standard of professional communication skills
