About the Role

We are seeking a highly motivated and dedicated Community Health Nurse to join our Unplanned Care Team. As a key member of our team, you will be responsible for delivering high-quality healthcare services to patients in their own homes.

Key Responsibilities
  • Work collaboratively with the multidisciplinary team to assess and manage patient needs
  • Develop and implement effective care plans to prevent hospital admissions and facilitate early discharges
  • Provide timely and high-quality care to patients in their own homes
  • Participate in the assessment of health needs and delivery of care to the practice population
Requirements
  • RGN qualification and current NMC registration
  • Ability to use Microsoft Office and Outlook
  • Awareness of current Health and Social Care policies
  • Previous experience of community healthcare (District Nursing) is desirable
