Job Description

We are seeking a skilled and compassionate Care Coordination Manager to join our team at Age UK Hillingdon, Harrow & Brent. This is an exciting opportunity for a leader who is passionate about delivering high-quality care services that make a real difference in people's lives.

Key Responsibilities:

  • Managing the day-to-day operations of the Hospital Discharge Support Service
  • Leading a team of staff and volunteers to deliver exceptional care to patients aged 18 and over
  • Liaising with other services to provide practical support for clients settling into their homes

Requirements:

  • A strong background in healthcare management or a related field
  • Excellent leadership and communication skills
  • The ability to work autonomously and as part of a team

Salary and Benefits:

Our Care Coordination Manager will receive a competitive salary of £32,000pa, plus a range of benefits including a contributory pension scheme, life insurance, and a generous holiday allowance.
