About the Role

We are seeking a highly skilled and dedicated Social Worker to join our Adults Services team at Devon County Council. As a Social Worker, you will play a key role in providing high-quality care and support to adults with complex needs, ensuring their safety, wellbeing, and independence.

Key Responsibilities
  • Conduct comprehensive care and support needs assessments, developing and reviewing personalized care plans in accordance with the Care Act 2014.
  • Support individuals to promote independence, applying an asset-based approach to optimize opportunities for reducing formal care and support needs.
  • Coordinate care and support arrangements, working closely with adults, carers, care providers, and other services to achieve desired outcomes and promote safety and wellbeing.
  • Complete mental capacity assessments and raise and triage safeguarding concerns, serving as the allocated practitioner for safeguarding enquiries.
