Medical Care Specialist

We are seeking a skilled Clinical Pharmacist to join our team at The Birches Medical Centre and Martlesham Heath Surgery. This exciting opportunity will enable you to make a positive impact on patients' lives by providing comprehensive acute and long-term support.
About the RoleAs a Clinical Pharmacist, you will work closely with local healthcare professionals to deliver clinical tools and resources that help manage patients with long-term conditions, medication requests, and acute issues. Your key responsibilities will include:
- To offer dynamic support to patients, focusing on their safety and well-being, and reducing hospital admissions.
- Management of related QOF indicators, including long-term condition medications, and involvement in implementing protocols and clinical guidelines.
- To identify opportunities for clinical reviews at individual practice, PCN, or CCG level where there is clinical need.
You will also be responsible for educating patients on their medications, enhancing awareness of alternative Clinical Services within primary care, and developing long-lasting relationships with practices and local NHS organisations.
This role offers a unique chance to utilise your specialist field knowledge and provide extended support to the practice and its patients.
RequirementsWe are looking for a highly skilled Clinical Pharmacist with:
- A minimum of 2 years current or recent clinical pharmacy experience.
- Ability to work effectively under pressure.
- A high level of medical and clinical English language skills.
