Job Summary

We are seeking an experienced Healthcare Support Worker to join our Community Care Teams in Harrogate and Rural area. As a Community Healthcare Support Worker, you will be based in one of our 4 Community Care Teams, providing high-quality care to patients in their own homes.

About the Role

As a Community Healthcare Support Worker, you will be responsible for assisting in assessing patients' health and social care needs. This will include taking and interpreting physical measurements and samples, monitoring changes in patients, and assisting in planning, delivering, and evaluating nursing care and rehabilitation programmes.
