About the Role

We are seeking an experienced Healthcare Support Worker to join our Community Care Teams in Harrogate and Rural area. As a Community Healthcare Support Worker with Harrogate District Foundation Trust, you will be based in one of our 4 Community Care Teams, providing high-quality care to patients in their own homes.

Key Responsibilities
  • Assess patients' health and social care needs, including taking and interpreting physical measurements and samples.
  • Monitor changes in patients and inform the team when changes might be significant.
  • Assist in planning, delivering, and evaluating nursing care and rehabilitation programs to address patients' health and social needs.
