Healthcare Support Worker

We are seeking an experienced Healthcare Support Worker to join our Community Care Teams in Harrogate and Rural area. As a Community Healthcare Support Worker, you will be based in one of our 4 Community Care Teams, providing high-quality care to patients in their own homes.
Key Responsibilities- Assess patients' health and social care needs, including taking and interpreting physical measurements and samples.
- Monitor changes in patients and inform the team when changes might be significant.
- Assist in planning, delivering, and evaluating nursing care and rehabilitation programs.
- A comprehensive induction program for all new staff members.
- Ongoing professional development opportunities, including access to local universities and training providers.
- A supportive environment to develop clinical skills, with a dedicated clinical educator.
- Access to a range of e-learning resources, training, and development opportunities.
- Support for career development and promotion opportunities within the Trust.
