About the Role

We are seeking a skilled Mental Health Practitioner to join our Assertive Outreach Service. As a key member of our team, you will be responsible for assessing, planning, delivering, and evaluating care for service users with complex mental health problems.

Key Responsibilities

  • Assess and manage risk, working within a psychosocial / family centred framework
  • Organise and plan your work as an autonomous practitioner, managing your time effectively and liaising closely with other agencies and disciplines
  • Undertake the role of Care co-ordinator/Lead professional for a case load of service users with complex mental health problems
  • Develop therapeutic relationships with service users to provide support and assistance
  • Administer and monitor the effects and adverse effects of prescribed medication, liaising with medical staff as necessary
  • Offer advice / information to clients and carers regarding prescribed medications
  • Communicate effectively with service users and carers who may be experiencing psychotic symptoms and be in distress
  • Undertake the role of Lead Clinician for people who are detained under a Community Treatment Order, completing formal tribunal/hospital managers reports as required

Requirements

  • RMN or Registered Mental Health Nurse qualification
  • Excellent assessment, risk management, organisational, and communication skills
  • Experience and a good working knowledge of the Care Programme Approach, Community mental health legislation, and Hospital admission / discharge process
  • The ability to use your initiative and manage your time effectively
  • The ability to work as part of a team and wider multi-agency services
