Bedded Units Medicines Optimisation Pharmacist - Band 7
We are seeking a dynamic and motivated pharmacist to join our expanding Medicines Management team at Central London Community Healthcare Trust. The successful candidate will provide a clinical pharmacy service at our bedded units within Hertfordshire & North London, supporting the delivery of high-quality patient care.
Main Duties of the Job
We are looking for a pharmacist to provide specialist expertise in the use of medicines, supporting the address of public health and social care needs of patients. The successful candidate will be involved in providing and developing a high-quality specialist Clinical Pharmacy Service across a range of inpatient settings in the Community Hospitals (Rehabilitation Units, Interim Care Beds, Palliative Care, Discharge to Assess ward) covered by the Bedded Services team.
The post holder will be responsible for providing support and guidance to Medicine Management Pharmacy Technicians, individualised patient pharmaceutical care, and a patient-focused pharmacy service to patients and staff. The successful candidate will also be involved in facilitating changes to patients' own use of medicines to improve safety, adherence, and empowerment.
About the Role
The successful candidate will join the existing team of Band 7 pharmacists at the Trust, covering a range of rehabilitation specialties. The post holder will be supported by experienced Senior Clinical Pharmacists, allowing them to gain new skills in each rotation and develop strategic, quality improvement, and leadership skills.
Requirements
- Vocational Masters Degree in Pharmacy
- Professional registration with the General Pharmaceutical Council (GPhC)
- Postgraduate Clinical Pharmacy Certificate qualification or equivalent
Desirable Qualifications and Experience
- Independent prescriber
- Primary and Community Health experience
- Experience of audit techniques
- Experience of training
- Experience of project / change management
