Experienced Nurse Required for Complex Care Role

We are seeking a highly skilled and enthusiastic NMC Registered Complex Care Nurse to join our Integrated Complex Assessment Team.
The successful candidate will manage a caseload of patients within the Discharge to Assess setting and be responsible for the completion of Continuing Healthcare assessments.
The role involves close working with Health and Social Care staff and requires knowledge of Community Home First Services alongside the principles of Discharge to Assess (D2A).
A valid driving licence and access to a car are essential due to the nature of the role, which requires adequate travel over a large geographical area.
The typical working hours are 8am-4pm, Monday to Friday.
Main Duties- Facilitate and develop individual programmes of care and care packages, providing specialist information and support to patients, their relatives, and other members of the caring team.
- Provide specialist nursing advice to the multi-professional team, promoting evidence-based practice to improve patient care outcomes.
- Monitor and improve standards of care through supervision, clinical audit, evidence-based practice, teaching, and supporting professional colleagues.
- Promote involvement and engagement of service users and their families in the assessment, planning, delivery, and evaluation of care.
- Support the healthcare team to provide a suitable learning environment, conducive to the acquisition of further skills and knowledge.
- Provide guidance and support to junior members of staff, complying with the NMC standards for mentors.
