Lead Practice Nurse

Whittington Moor Surgery is seeking an experienced and skilled Lead Practice Nurse to join our team. As a key member of our multidisciplinary team, you will be responsible for the management and clinical supervision of the practice nursing team, ensuring the delivery of high-quality patient care.
Key Responsibilities:- Act as the Infection Prevention Control (IPC) lead, ensuring compliance with IPC policies and procedures.
- Develop and implement health promotion and wellbeing programmes to promote healthy lifestyles and disease prevention.
- Assess the needs of patients and provide evidence-based care, incorporating chronic disease management and health promotion strategies.
- Develop and implement individual treatment plans for chronic disease patients, promoting health and wellbeing.
- Identify, manage, and support patients at risk of developing long-term conditions, preventing adverse effects on their health.
- Provide routine nursing care to patients, adhering to clinical guidelines and best practices.
- Diagnose and manage acute and chronic conditions, prescribing medication as necessary.
- Coordinate the provision of travel medicine services and oversee the management of chronic disease clinics.
- Maintain accurate clinical records, ensuring compliance with legislation and regulations.
- Experience of working in a primary care environment, with a focus on chronic disease management.
- Registered Nurse (Nursing and Midwifery Council) qualification.
- GCSE grade A to C in English and Maths.
- Family planning qualification.
- Desirable: Mentor or teaching qualification, postgraduate diploma or degree in Chronic Disease Management, completed or working towards formal infection prevention and control training, and nurse prescriber status.
