Lead Practice Nurse

Whittington Moor Surgery is seeking an experienced and skilled Lead Practice Nurse to join our team. As a key member of our multidisciplinary team, you will be responsible for the management and clinical supervision of the practice nursing team, ensuring the delivery of high-quality patient care.
Key Responsibilities:- Act as the Infection Prevention Control (IPC) lead, ensuring compliance with IPC policies and procedures.
- Develop and implement health promotion and wellbeing programmes to promote healthy lifestyles and prevent chronic diseases.
- Assess the needs of patients and provide evidence-based care to ensure the provision of appropriate healthcare services.
- Develop and implement individual treatment plans for chronic disease patients to promote health and wellbeing.
- Identify, manage, and support patients at risk of developing long-term conditions, preventing adverse effects on their health.
- Prescribe medication for patients in accordance with PGDs and own scope of practice.
- Review patient medications to enhance compliance and provide guidance on the use of prescribed and over-the-counter medications.
- Co-ordinate the provision of travel medicine services and oversee the provision of chronic disease clinics.
- Maintain accurate clinical records and ensure the use of read codes to facilitate easy and accurate information retrieval.
- Experience of working in a primary care environment, preferably as a practice nurse or community nurse.
- Experience of chronic disease management and infection prevention and control measures.
- Experience of working autonomously and as part of a multidisciplinary team.
- Registered Nurse (Nursing and Midwifery Council) and GCSE grade A to C in English and Maths.
- Family planning qualification and mentor or teaching qualification desirable.
