Job Summary

We are seeking an experienced Clinical Pharmacist to join our multidisciplinary team. The successful candidate will have the opportunity to work closely with GPs, pharmacists, nurses, and other healthcare professionals to deliver high-quality patient care.

Main Responsibilities

  • To work as part of a multi-disciplinary team to support delivery of the new network Directly Enhanced Service (DES)
  • Conduct structured medication reviews to proactively manage patients with complex polypharmacy
  • Provide clinical medicines expertise to both patients and practice teams
  • Improve prescribing practices across the PCN through clinical audits and quality improvement initiatives
  • Interface with community and hospital colleagues to resolve medication-related issues
  • Assist with delivery of enhanced health in care homes initiatives

Clinical Administration

The successful candidate will be responsible for:

  • Checking and reviewing clinical post
  • Medicines reconciliation
  • Medication queries and requests
  • Signposting
  • Authorising repeat prescriptions
  • Interpreting pathology and signing prescriptions (if qualified)

Clinical Delivery

The successful candidate will also be responsible for:

  • Clinical medication reviews
  • Polypharmacy reviews
  • Care home support
  • Care planning
  • Minor illness clinics (if qualified)
  • Triage

Practice Process and Performance

The successful candidate will contribute to:

  • Best practice prescription management
  • QOF optimisation and prevalence finding
  • CQC support
  • Local incentive scheme support
  • Opportunities for increasing practice revenue
  • Clinical audits

Person Specification

The ideal candidate will have:

  • Registration with the General Pharmaceutical Council
  • Evidence of Continuing Professional Development
  • Good critical appraisal skills
  • Experience of patient consultation
  • Good understanding of primary care structure and General Practice
  • Good clinical knowledge

Desirable Qualifications and Experience

  • Non-medical prescriber registration
  • Advanced assessment and history taking skills, or willingness to develop these
  • Post-registration experience in primary, secondary, or community pharmacy
  • Experience of clinical pharmacy
  • Knowledge of GP clinical computer systems

Salary

The salary for this role is approximately £55,000 - £65,000 per annum, depending on experience.
