About the Role

We are seeking an experienced Advanced Clinical Practitioner to join our team at The Chislehurst Partnership. As a key member of our healthcare team, you will be responsible for providing high-quality patient care and contributing to the development of our clinical services.

Key Responsibilities
  • Work collaboratively with our multidisciplinary team to deliver patient-centered care
  • Conduct thorough clinical assessments and develop effective treatment plans
  • Prescribe and review medication, ensuring safe and effective use
  • Provide education and support to patients, promoting self-management and health literacy
  • Participate in quality improvement initiatives, contributing to the development of our clinical services
  • Collaborate with external stakeholders to ensure seamless delivery of care
Requirements
  • Registered with the NMC and a postgraduate qualification at Level 7 or above
  • Experience working autonomously in an advanced nursing role, with a strong understanding of clinical governance
  • Excellent communication and interpersonal skills, with the ability to work effectively in a team
  • Ability to prioritize workload efficiently and exercise sound judgment in complex situations
